We created this page to help you prepare for your first visit. You will find all the documents you need to complete before your first visit, directions to our office and answers to some frequently asked questions. Please contact us if you should need further assistance.
Step 1: Paperwork
When visiting us for the first time, we ask you to fill out forms so that we may better serve you. As with any office, we request that you come in a few minutes early to complete the forms. To save you time, we added the forms to this page so they can be completed at your convenience. Before your first visit, please click on the links below to download the new patient forms. Then print the forms, fill them in completely, and bring them with you to your first appointment. By completing this paperwork ahead of time, we save you time and you help us see you on time and allow other patients to be seen on time, as well.

Cancellation / No-Show Policy: We understand that things happen in life that may force you to change your plans. We request that you contact our office at least 24 hours before your scheduled appointment. If you do not show up for your appointment or call less than 24 hours before your appointment, you may be charged a cancellation/no-show fee.

Our office is located at the Alpine Village Center on the way to Big Mountain (Whitefish Mountain resort) approximately one mile north from the turn at highway 93 onto Baker Ave. Baker Ave. becomes Wisconsin Ave. when crossing the Rail Road bridge. We are located on the right side of Wisconsin Ave. just north of the Alpine Grocery.

ANSWER: Our payment options are as follows. We accept cash, debit card, checks, MasterCard, VISA and Discover. We also offer immediate application processing for Care Credit which provides low monthly payments with an interest free option.
QUESTION: Are There Any Hidden Costs?
ANSWER: No. We do our best to outline all costs up front. The cost for your first visit is discussed when you call to make an appointment. If more treatment is necessary, we will discuss the cost and procedures before we proceed.
